What is Care Coordination?
Canvas Health’s care coordinators work with our clients to thoughtfully organize services, providers, and resource access in ways that most effectively improve their overall health and functioning.
Care coordination includes working with multidisciplinary teams and psychosocial support providers internally and throughout the community.
What are the goals of Care Coordination?
CCBHCs are responsible for providing care coordination to all clients as part of their person and family-centered way of managing care, which involves organizing care activities among different services and providers and across various facilities.
Services are particularly important and impactful for clients who have complex mental and physical health care needs as well as educational support needs. Care coordinators ensure clients obtain referrals and resources needed to manage their mental health and physical health needs and do not fall through the cracks or get overwhelmed in the process. Your Care Coordinator will walk alongside you to assist with scheduling appointments, completing paperwork, finding community resources, and much more.
Who is involved in Care Coordination?
Canvas Health care coordinators can work with an individual or their family while enrolled in a CCBHC service at Canvas Health. They will work closely with clients and their families to understand their needs and preferences so that information can be used to communicate at the right time to the right people, including mental health, physical health, educational, and other social services. This information is used to provide safe, appropriate, and effective care to the patient and their immediate family.

Care Coordinator Supervisor
Working every day to help decrease barriers to accessing care related to mental health, physical health and basic needs. Collaboration within the agency and throughout the community will benefit all involved. Utilize lived experience and almost two decades of continued training around trauma informed care, cultural competency and genuine compassion, to do all I can to ensure clients and their family (whatever that looks like for them) can access resources that will positively impact their life.
I have worked in mental health care for more than 15 years. Experience in high intensity residential treatment center, day treatment for youth, and the past eight years in outpatient care coordination.

I have worked in the mental health field for 17 years in a variety of settings that include Community Behavioral Health Hospital, IRTS, and the past 11 years working as an Adult Rehabilitation Mental Health Service provider.
